Cerebrovascular Risk Factors and Altered Brain Structure in Healthy Individuals

Cerebrovascular disease (CVD) is the third leading cause of death in both Hawai’i and the entire U.S. Diabetes, dyslipidemia, hypertension and other cardiometabolic disorders, which are over-represented in the Native Hawaiian population, are risk factors not only for myocardial infarction and stroke but for vascular dementia and mixed vascular dementia/Alzheimer’s Disease. Utilizing Hawaii-wide hospital discharge records, we recently identified a substantial health disparity in dementia prevalence, and in particular of vascular dementia, among Native Hawaiians. Determining the relationships between cognitive dysfunction and specific cerebrocardiovascular risk factors such as hypertension, cholesterol, and diabetes will prove crucial to the planning of preventive and treatment strategies and ultimately to understanding the mechanism that underlies CVD-related dementias. Vascular neurocognitive impairment in CVD reflects damage to global gray and white matter, especially in the deep gray nuclei and frontal cortex. This RMA TRIX project proposes to examine structural brain changes cross-sectionally in 50 ‘healthy’ middle-aged and elderly adults (age ≥ 40 years) who have varying degrees of CVD risk factors. Specifically, we will use magnetic resonance imaging (MRI) to assess alterations in regional cortical thickness, a sensitive measure of macrostructural gray matter change. We will also employ a newer neuroimaging modality, diffusion tensor imaging (DTI), to examine microstructural abnormalities in white matter and subcortical gray matter regions. Associations between individual cerebrovascular risk factors and regional brain tissue integrity, both macrostructural and microstructural, will be investigated with these two
techniques.

Progress
METHODS: Using MRI and Diffusion Tensor Imaging participants will be evaluated for CVD risk factors. Magnetic resonance imaging (MRI) will be used to assess alterations in regional cortical thickness, a sensitive measure of macrostructural gray matter change; and using a newer neuroimaging modality, diffusion tensor imaging (DTI), microstructural abnormalities in white matter and subcortical gray matter regions will be examined.

CONCLUSIONS: Data are currently being analyzed that suggest CVD risk factors may have unique characteristics in the brain.
